Substance Use Disorder Services and Programs

Assertive Community Treatment (ACT) is a program that provides complete, community-based care for people with substance use disorders who haven’t found enough relief from other kinds of treatment. ACT is designed for those with the most severe symptoms and the greatest challenges in daily life—such as meeting basic needs, finding safe housing, and staying safe.

Adults in ACT receive support from a dedicated team of professionals, including mental health specialists, nurses, and psychiatrists. Our Riverwood Center ACT team has training in psychiatry, social work, nursing, substance use disorders, and job training. They work together to provide services that may include:

• Medication management
• Supportive therapy
• Crisis intervention
• Hospitalization
• Substance use disorder treatment, including group therapy
• Life skills training, such as managing daily activities
• Help finding and keeping a job
• Support with legal and advocacy services, financial aid, housing, money management, and transportation

Case Management at Riverwood Center supports adults in recovery by helping you build confidence and independence while managing the challenges of substance use and daily life. As part of your recovery journey, our team provides ongoing, coordinated care to ensure you have support every step of the way.

Your Case Manager will work closely with you to understand your unique needs and connect you to resources that make a difference—like assistance with finances, food, housing, medical and dental care, transportation, social connections, and counseling. We also support individuals in hospitals, jail, or other restricted settings.

Because every recovery journey is different, our Case Management services are tailored to you and can be offered in the office or out in the community. No two paths are the same, but we’re here to walk alongside you as you move toward a healthier, more hopeful future.

Community Living Supports (CLS) at Riverwood Center are designed to help adults in recovery from substance use disorders live more independently and be actively involved in their community. Our specially trained staff will work with you based on your personal goals, choices, and priorities identified in your individual recovery plan. Supports can be provided in your home or in the community—wherever you need them most.

You may receive help with things like:

• Cooking healthy meals
• Doing laundry and keeping your home clean
• Help with daily tasks like bathing, eating, dressing, and self-care
• Grocery shopping and other daily errands
• Getting involved in social and community activities
• Managing money and daily budgets
• Building and maintaining healthy relationships
• Transportation to and from appointments and activities
• Joining community events or recreational activities
• Attending medical appointments
• Accessing needed items and non-medical services
• Medication reminders and wellness check-ins

Our goal is to support your independence and help you take steps toward a healthier, more connected life in recovery.

This group is designed for individuals who are new to treatment—whether they feel uncertain about their substance use or are seeking help at the request of an employer, the legal system, or family members. Throughout this eight-week group, participants meet once a week to explore their relationship with substances and gain a deeper understanding of addiction and the recovery process. By the end of the group, our hope is that each person will feel more informed about their substance use and more prepared to take positive steps toward lasting change.

Supported Employment helps those who are overcoming the challenges that a mental illness can have when finding and retaining a job. Riverwood Center Employment Specialist guide individuals through the job search process to assist individuals with finding competitive employment that meets their needs and preferences. Employment Specialist continue to work with individuals after a job is obtained, by providing on the job and follow along supports as needed, until individuals feel comfortable to work independently and graduate from services.

Supported Employment offers assistance to individuals looking to further their career by helping individuals explore educational and vocational opportunities in their communities.

The Intensive Outpatient Program (IOP) is designed to help participants focus on sobriety and work through the early stages of recovery. IOP meets for three hours a day, three days a week, with both morning and evening sessions available to fit your schedule.

Participating in IOP offers many benefits:

• Peer support and encouragement to stay substance-free
• A sense of connection that helps reduce feelings of isolation often experienced in recovery
• Opportunities to witness others’ progress, inspiring hope and motivation
• Shared learning, as group members see and discuss different ways of coping with substance use and life challenges
• Helpful information for those new to recovery
• Constructive feedback from peers about strengths and areas for growth
• Encouragement, coaching, and support as you take on new or challenging steps in your recovery journey

IOP is a supportive environment where you can build skills, gain confidence, and connect with others who understand the path you’re on.

We offer Medication-Assisted Treatment (MAT) as part of our whole-person approach to substance use recovery. MAT combines carefully chosen medications—like buprenorphine, which is approved by the FDA to treat opioid use disorder—with counseling and other supportive therapies. This combination helps reduce cravings and withdrawal symptoms, making it easier to focus on your recovery and overall well-being.

MAT is just one part of a comprehensive treatment plan. Our Riverwood Center team works closely with you to make sure your care fits your individual needs. To determine the best level of care, we use the American Society of Addiction Medicine (ASAM) Criteria. This assessment looks at important areas of your health and life, such as:

• Your potential for withdrawal
• Any medical needs or complications
• Emotional, behavioral, or cognitive health
• Readiness and motivation for change
• Risk of relapse or ongoing substance use
• Your living and recovery environment

Our goal is to support you with evidence-based, compassionate care—helping you move forward on your recovery journey with the right mix of medical support, counseling, and community resources.

We offer a Motivational Enhancement Group, designed to help individuals in recovery find and strengthen their own reasons for change. Guided by compassionate Riverwood Center team, this group provides a supportive space where you can explore your goals, build confidence, and overcome ambivalence about recovery. Through open discussion, personalized feedback, and encouragement, you’ll gain practical tools and insights to move forward at your own pace. Whether you’re just beginning to consider change or looking to maintain your motivation along the way, Riverwood Center’s Motivational Enhancement Group is here to support your journey to a healthier, more hopeful future.

*Participants take part in a 5-session group, designed to support and motivate their recovery journey. After completing these sessions, each individual is referred to the next level of care or service that best meets their recovery needs.

Recovery Management (RM) is here to help you stay on track with your recovery long after initial treatment ends. Our RM services provide ongoing support, encouragement, and resources to help you maintain the progress you’ve made and address any challenges that come up along the way. Whether you’re facing new stressors, need help connecting to community resources, or just want someone to check in as you continue your recovery journey, our Riverwood Center team is here for you.

Recovery is an ongoing process, and with Riverwood Center, you don’t have to do it alone—we’re committed to supporting your success every step of the way!

Our Riverwood Center Relapse Prevention and After Care group is designed to support individuals who have achieved at least 30 days of sobriety and are ready to focus on maintaining their progress. This weekly group offers ongoing support as you continue your recovery journey, providing a safe and understanding environment where you can build confidence and develop lasting skills.

The program is grounded in the understanding that relapse doesn’t happen overnight—there are often predictable warning signs and patterns that come first. By learning how to recognize and manage these early warning signs, you can take proactive steps to protect your recovery.

In this group, participants work together to:

• Identify personal triggers and high-risk situations
• Build healthy coping strategies for dealing with stressors
• Strengthen and expand their support network
• Recognize their own strengths and successes
• Share experiences and encouragement with others facing similar challenges

Throughout your time in the group, you’ll develop personalized strategies and goals to help you navigate the ups and downs of recovery. Discharge from the group is individualized and occurs when you have met the goals of your treatment plan and feel confident in your ability to maintain sobriety.

With Relapse Prevention and After Care at Riverwood Center, you’ll have ongoing guidance and support to help you stay on track, manage life’s challenges, and celebrate your continued growth in recovery.

At Riverwood Center, we believe that self-determination is a vital part of your recovery and well-being. Our self-determination services are designed to give you greater choice, control, and flexibility in your care. This means you can play an active role in creating your own person-centered plan, selecting the services and supports that best fit your needs and goals. We’re here to empower you to make decisions about your treatment, manage your own services and budget (when eligible), and build the skills and confidence you need to live life on your own terms. With Riverwood Center’s support, self-determination puts you at the center of your care—every step of the way.

Under Self-Determination, you can hire your own workers and manage your services within a set budget.

  • You have choice and control over your services.
  • Your Case Manager/Supports Coordinator supports you to manage the services you choose.
  • You are the employer of record with a Fiscal Intermediary (someone who pays your worker(s) for services).
  • You directly hire, fire and supervise your workers.
  • You set your workers' schedule.
  • You decide what to pay your workers (within Riverwood Center's rate).
How does Self-Determination work?
  • You make choices about your goals when creating your Person-Centered Plan.
  • You determine who you want to help you reach your goals.
  • Your family and friends can help you with decisions to make sure that your health and safety needs are met.
  • After you have a plan, we will develop a budget based on those services.
  • You manage your individual budget (with help from your Case Manager/Supports Coordinator and Fiscal Intermediary).
How do you begin Self-Determination?

It starts with your Person-Centered Plan. At your next Person-Centered Plan meeting, tell your Riverwood Center Case Manager or Supports Coordinator that you are interested in Self-Determination. You can also contact our Self-Determination Coordinator at 269-934-1655.

Important to know:

Medicaid rules still apply under Self-Determination. That means:

  • Services must be medically necessary:
    • The kind, amount and length of service must be supported by your condition.
    • Your condition would worsen without the services.
    • All other natural and community supports must be used before Medicaid will pay for a service.
    • Services must be supported by documentation.
  • The services provided must match what is in your Person-Centered Plan.
  • The staff you choose to hire must meet the minimum Medicaid requirements:
    • At least 18 years old
    • Able to prevent transmission of any communicable disease from self to others
    • Able to communicate clearly with you and Riverwood Center staff
    • Able to report on activities performed
    • In good standing with the law according to the MDCH/PIHP contract
    • Complete required trainings, which may vary based on the type of services being provided

At Riverwood Center, we use the American Society of Addiction Medicine (ASAM) Criteria to help determine the best level of care for your substance use treatment and ongoing support. This approach ensures that your care is truly personalized and meets your unique needs at every stage of recovery.

The ASAM Criteria considers six important areas of your health and life:

• Potential for withdrawal
• Any medical conditions or complications
• Emotional, behavioral, or cognitive needs
• Readiness and motivation for change
• Risk of relapse or continued substance use
• Your current living and recovery environment

Our substance use and co-occurring therapy services are designed to help you build healthier coping skills, strengthen self-control, and restore emotional and psychological well-being. We use evidence-based practices that address both substance use and co-occurring mental health needs—helping you feel more confident, connected, and ready for positive change in your life and relationships.

For individuals who need more intensive support, Riverwood Center offers Higher Level of Care (HLOC) services for substance use disorders. HLOC provides structured, comprehensive treatment in a safe environment, designed to address complex needs and support your recovery journey. Our team works with you to develop a personalized care plan, offering a combination of medical support, counseling, and therapeutic services to help you stabilize, build new skills, and take meaningful steps toward lasting recovery.

Our Women’s and Families Speciality Services (WFSS) program is dedicated to empowering mothers and pregnant women who are facing challenges with alcohol and drug use. Our goal is to support women and their families in building healthier, more stable lives—every step of the way.

Our program is built around these core goals:

• Helping women establish and maintain sobriety
• Reducing barriers to treatment, including assistance with transportation, child care, and housing, as well as connecting you to healthcare and community resources
• Providing advocacy and support with all involved providers, such as courts, CPS, foster care, healthcare, and medication-assisted treatment
• Supporting the overall well-being and functioning of the family system

At WFSS, we practice unconditional care—meaning we meet families where they are and support the goals that matter most to them. Our services and programming adapt to the unique needs of each family, ensuring you receive the support that fits your journey. Services available through WFSS include:

Case Management: We help with family planning, employment, housing, budgeting, parenting skills, food and other benefits, and removing barriers to services—helping families move toward greater self-sufficiency.

Group Treatment: Our groups are designed to strengthen recovery and personal growth, addressing topics such as trauma, abuse, and violence, while recognizing and building on each woman’s strengths.

Individual Therapy: Focused on the importance of relationships in women’s lives, our therapists help you navigate challenges and build on your unique strengths from a relational perspective.

Parenting and Life Skills Classes: Practical support to help families thrive.

Integrated Care: Women who are part of WFSS and also experience mental health challenges will receive coordinated substance use and mental health services tailored to their needs and preferences.
