Get Your Intake Appointment

During your first call, we’ll schedule a convenient intake appointment. At your intake appointment, our team will take time to understand your needs and goals, so together we can create a care plan that connects you with the right services and supports for your journey. From there, you’ll begin receiving the care and support that’s right for you, guided by professionals dedicated to your health and wellness.

From the very beginning, we’ll listen to your concerns, answer your questions, and include your voice in creating a plan of care that fits your needs. What To Expect

Getting services at Riverwood Center is simple. Our Access Center is open for walk-ins or phone calls Monday-Friday, from 8:30 a.m. to 5:00 p.m. If you are a walk-in and arrive after 3:30 p.m., we may need to finish your intake on another day that works best for you as it could take 1.5-2 hours.

When you come in, one of our friendly Access Specialists will sit down with you and ask some questions. We’ll talk about where you live, what’s going on in your life right now, and how services might be helpful. We’ll also ask about insurance, income, and basic information so we can connect you with the right care. Together, you and the Access Specialist will decide on the next step:

If it’s an emergency, you’ll be connected to immediate help.
If it’s not an emergency, and you’re eligible for services, we’ll connect you to the right provider. This process usually takes 1.5 to 2 hours, and by the end you’ll know what services you qualify for. If eligible, you’ll also set up an appointment with one of our clinicians.
If you’re not eligible, we’ll help guide you to other community resources.
If you have private insurance, you may need to start with one of your insurer’s providers before receiving services at Riverwood Center.

To help us serve you more quickly, please bring:
• Your medical insurance card or numbers (including Medicaid, if you have it)
• Your Social Security number
• A list of current medications and doctors
• Proof of guardianship, custody, or other legal documents (if this applies to you)
• School records or special education plans (if this applies to you)
• Proof of income (like check stubs, W-2s, or tax returns)

For children and teens:
Children under 14 must have a parent or legal guardian with them to give written permission for counseling.
All children under 18 must have a parent or legal guardian give permission for any medication.

If someone has a legal guardian (child or adult), the guardian must attend the intake appointment and bring the paperwork that proves guardianship.

Eligibility

As a CCBHC, we provide care to individuals of all ages with both mild and severe behavioral health conditions. Services are available to individuals who:
• Live in Berrien County or want to travel to Berrien County for support.
• Have a serious mental illness, developmental disability, serious emotional disturbance, substance use disorder and/or co-occurring substance use disorder.
• Have Medicaid. (*If you do not have insurance, we can provide you with a Medicaid application. If you do not have insurance and meet all other eligibility criteria, you may be placed on a waiting list.)
• Demonstrate medical need for services.

If you do not qualify for services or support, a member of our Riverwood Center Access Team will assist you in finding or referring other services available to you.

Referrals

We accept referrals from many sources, including primary care and other health providers, law enforcement, attorneys, the Department of Health and Human Services (DHHS), local shelters, schools, and employers. Individuals may also come to us through self-referral or with the support of a family member.

Advance Directives

Advance Directives give you the opportunity to make decisions about your future care before you ever need it. At Riverwood Center, we encourage individuals to create an Advance Directive as part of their wellness planning when getting started with care.

This document allows you to share your wishes about treatment and choose a trusted person to speak on your behalf if you are ever unable to do so. Having an Advance Directive in place helps ensure your voice is always heard, your preferences are respected, and your Riverwood Center support team can honor your choices. If you’d like more information or help in creating an Advance Directive, our Customer Service team is here to guide you.

Payment for Services & Non-Discrimination

At Riverwood Center, we provide health care services to all individuals and do not discriminate:
• If someone is unable to pay for services
• If payment comes from Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP)
• Based on race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation

We work with each person to understand their needs and determine what services may be covered. No one will be denied access to services because of inability to pay. A discount/sliding fee scale is available, based on family size and income.
